All Athletes Must Be Cleared - Here's How

Girls are not allowed to compete for their school in games until they have their Physical and Athletic Clearance, so we need ALL GIRLS done with this process by the end of September. The Clearance is completed online - see Instructions/Forms by clicking on the link in #1 below (Download...).. A physical must be completed and a form signed by a doctor each year.  After that, the player must be cleared to participate through the following process:

1. Download the Athletic Clearance Instructions/Forms (click link)
2. Register at the Athletic Clearance Website - Home Campus (click link)
3. An athlete must be cleared before participating in any sport.

Athletic Clearance – Required Documents

•  1. Confirmation Message from homecampus.com signed and dated (example below) *MUST BE SIGNED AND SUBMITTED TO ATHLETICS OFFICE IN ORDER TO BE CLEARED (Please either email to [email protected] , or bring to NH Athletics Office) 
• 2. Pre-Participation Exam Form signed and stamped by a physician – (upload to homecampus) *Physicals are good for one year after your exam date – please make sure it is dated and signed by the physician. 
• 3. Current medical insurance card (upload to homecampus) *Medical insurance is required to participate in athletics. If you currently do not have medical insurance and would like to purchase, please see the Athletics Office for options. *If you cannot upload your physical and insurance card, you may turn it in with your signed Confirmation/Consent Page to the Athletics Office*
